We all collect and report a lot of data, but how do we know if our patients are getting healthier? Without a system to understand the health of the patients we treat, it can be difficult to see the fruits of our labors. During this webinar, the DentaQuest Partnership will discuss how we’ve applied Quality Improvement and measurement to caries management, and how your dental practice can begin implementing some of these strategies to help monitor outcomes among your patient population.

During this webinar participants will learn about the DentaQuest Partnership’s approach to interprofessional practice and the MORE Care (Medical Oral Expanded Care) program. MORE Care is an initiative of the DentaQuest Partnership focused on providing primary and secondary preventive oral health services to underserved populations in primary care medical offices, as well as testing optimal patient-centered referral systems between primary care and dental care teams.
Participants will also hear about MORE Care efforts in Pennsylvania and primary care clinics’ experience with integration and coordination of oral health care. The presenter will discuss themes that developed, lessons learned, and some key facilitators and challenges.
During this webinar the DentaQuest Partnership provides an overview of social determinants of health (SDOH) as they relate to oral health and discuss how we incorporate SDOH into the work we do as a grantmaking organization. Several DentaQuest Partnership grantees also discuss how this work has come to life on the national, state and local levels.
By the end of this webinar, participants will be able to:
1.Describe social determinants of health as they relate to oral health.
2.Describe how providers, community based organizations, and other local, state, and national partners can use a social determinants of health lens to address health disparities.
3.Identify potential strategies that can be implemented to address social determinants of health on the national, state, and local levels.
