Integrate oral health into person-centered healthcare

Building on the work of a three-year HRSA grant, the team will demonstrate financial sustainability of an integrated care model to promote comprehensive, person-centered care and improve access to care by placing a nurse practitioner in the dental office. $125,800 will support staff time, site visits, and technology costs. This proposal aligns with Oral Health 2020 Goal 5: Integrate oral health into person-centered healthcare.
